Judy Waterlow MBE, designed and researched this pressure ulcer risk assessment tool in 1985, while working as a Clinical Nurse Teacher.
The Waterlow pressure ulcer risk assessment/prevention policy tool is, by far, the most frequently used system in the U.K. and it is also the most easily understood. Intended for use by nurses, healthcare professionals and carers at the patient/client interface.
It must be remembered that this, like all risk assessment scoring systems, is a simplistic tool. Professional judgement must be used in determining the risk status of the service user at all times.
However, the outcome of the assessment does provide you with the tools and knowledge to put preventative measures and treatment plans in place.
